All Action 2000 wheelchairs produced by Invacare with serial numbers from 7007533957 to 7008568013 have had a medical device alert placed upon them.

The bolts used to secure the castor assemblies to the wheelchair frame can fail in use. This has caused occupants to fall from their wheelchair and sustain injuries. So, if you are involved in the provision, prescription, repair, maintenance and use of these wheelchairs. In particular: wheelchair service managers, rehabilitation engineers, occupational therapists, maintenance staff or contractors, then you need to  

  

*  Trace all Action 2000 wheelchairs in the specified serial number range.

* Obtain the replacement bolt kit, reference 1528509 from Invacare (free of charge).

* Ensure the bolts are replaced in accordance with the instructions.
