Faulty raised lavatory seats has led to at least six people being injured, according to the MHRA.
One woman died, but it was not clear to what extent the fall had contributed to her death.
The Medicines and Healthcare products Regulatory Agency (MHRA) said it had received 91 reports of broken Homecraft Rolyan Savanah seats from the 25,000 that have been sold.
The MHRA say that six people have fallen off the toilet because the seat had slipped. Injuries include bumps to the head, bruising and back injuries. It is urging the public as well as health and social care providers to be on the look out for the potential dangers of the raised seats.
The product codes are AA2112 and AA2112L for the two-inch seats and AA2114Y and AA2114L for the four-inch seats. So check your stock.
The problem lies in the rubber pad which is used to secure the seat. It comes away from the bracket.
People can report any safety related issues concerning medical devices to the MHRA by calling 0207 084 3080 or visiting www.mhra.gov.uk
